Modern view of perioperative pain is to it as an impediment recovery and various methods are used facilitate rapid return normal functional activity. Post operative relief after knee surgeries, especially total replacement, a major concern. It severe in 60% patients moderate 30%. causes undue distress hinders early intense physical therapy, considered one the most important factors for optimal postoperative rehabilitation. The present study compares efficacy between periarticular cocktail epidural analgesia post management following TKA. A 78 were included study. By simple randomization divided into Group which 48 received B 30 Epidural top up dose. -A analgesic 0.125% Bupivacaine (2mg/kg body weight), Ketorolac (30mg), Adrenaline. This 20 cc was infiltrated posterior capsule, medial structure, lateral structures cut quadriceps tendon before placing implant. topup 8cc given 6 hours surgery patients. No drain placed Periarticular infiltration group. straight leg raise test done assess VAS score at 12h, 24h, 48h, 72h. provides effective control despite disadvantages its invasiveness, late motor functions more systemic side effects. Local infiltrative should be due ease use, lower rates effects fact that does not extend block or require application LA, while providing comparable equal range movements ambulation immediate period
